Description: Summit Educational Resources is seeking to fill the remainder of a three year contractual fee for Service position for a Nutritional Consultant, to service children with developmental disabilities. The consultant will be utilized 160 hours/year for Years 1 & 2, and 80 hours/year in Year 3. Duties will include conducting nutritional assessments, targeting nutritional needs, developing individual dietary plans or programs based upon the needs of a the children, and counseling the individual child, classroom staff, and caregivers. The position will also involve instruction and follow-through on individual plans, charting progress, and nutritional education for parents and staff by conducting in-service trainings for staff and parents on nutritious meal planning, weight management, proper nutrition and weight loss plans.

Description: Heritage Christian Services is looking for caring people to join our team. Heritage Christian Services is a thriving human services agency supporting over 1,400 children and adults with developmental disabilities through the operation of 53 residential homes, service coordination, respite and 18 day habilitation programs. We were inducted into the COMPASS program as one of New York State's top service providers to people with special needs.
Responsibilities: Weekly visits to primary homes for more medically needy individuals, and visits to secondary and consulting homes on a less frequent basis. Yearly nutrition in services for staff at primary homes. In-services on an as needed basis at residential homes and dayhabs. Menu development that includes a 5 week cycle menu for the homes for fall-winter and spring-summer. This includes the preparation of grocery shopping lists, as well as recipe cards and a breakdown of staff job responsibilities for cooking to ensure the nutritional requirements of the individuals in the homes are being met. Meet with individuals that live in the homes to discuss weight loss, reading food labels, proper nutrition, and/or portion sizes on an as needed basis. Menu planning for other homes, especially the apartment settings. Review drug nutrient interactions. Initiation of staff supports and diet guidelines for therapeutic diets, and as needed for regular diets Issue verbal telephone orders for diet changes, communicate with other members of the healthcare team for issues/concerns that arise. Med course nutrition training 2 times per month in Rochester.Initiation and monitoring of enteral feeds, either G or J tubes.
Qualifications: The successful candidate must possess a Bachelor's Degree. Minimum 2-3 years experience Ability to be self-directed, highly organized and proactive. Excellent listening skills. Ability to communicate with all levels of the organization. Effective oral and written communication skills. Ability to effectively present information to audiences with a variety of knowledge/skill levels.

Description: Crandall Corporate Dietitians currently has an opening for an RD consultant for Assisted Living communities in Brockport, Fairport, Lakewood, Liverpool, Painted Post, Rochester, Syracuse, Vestal and Williamsville, NY. These visits need to be during normal business hours, Monday thru Thursday, 8 am to 5 pm. This is a part time position requiring 4 hour visits semi-annually. During the semi annual visit, you will provide dietary consultation to high nutrition at risk residents, observe a meal service, speak with residents and conduct a kitchen sanitation audit. Crandall provides all the materials you will need: manuals, report forms, etc. We will have our Corporate Trainer contact you to go over the materials in a conference call training. We do require RD to carry professional liability insurance, current CDR and current TB test. Mileage reimbursement is provided for travel =25 miles one way, =50 miles round trip.
